Please print and complete the attached Registration Form. Upon receipt, we will mail you a donor card which should be carried at all time. This form and the card are legal documents in all 50 states under the Uniform Anatomical Gift Act and similar laws. If you cannot print the Donor Registration Form, please select the Registration Request link and let us know. We will mail you a donor registration form and other related documentation.
We encourage you to discuss your decision to be an organ donor with your family members so that, when the time comes, they will fully understand your desire to be an organ donor.
